Your information, handled simply.

When you book a consultation, we collect the information you enter into the booking form: your name, phone number, email address, ZIP code, any message you write, and any files you choose to upload. We also record the date and time of your appointment.

We do not collect Social Security numbers, full dates of birth, or bank and card details through this site. Please do not enter those into any field.

We use this information for a single purpose: to prepare for and hold your consultation. That includes confirming your appointment by email, adding it to the broker’s calendar, and reviewing any documents you upload before you speak.

If you decide to enroll in a plan, we may share the details relevant to that plan with the carrier you choose. We do not share your information with anyone else, and we do not sell it.

Booking records are stored in a secured database. Uploaded documents are stored in private cloud storage and are only accessible to the licensed agent handling your appointment.

We keep booking records and documents for as long as needed to serve you and to comply with applicable record-keeping requirements. When you ask us to delete your information, we will — except where we are legally required to keep a specific record.

This site does not use tracking cookies, advertising pixels, or third-party analytics by default. The only browser storage we use is temporary session storage to render your booking confirmation and it clears when you close the tab.

This site is not directed at children under 13, and we do not knowingly collect information from them.

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. You can also withdraw your consent to be contacted at any time. Reach us at syliviaa@choiceinsassociates.com or +1 (318) 787-1552.

If we update this notice, we will change the date at the top of the page. Significant changes will be announced on this site.
